External Referrals
When a patient needs care beyond your facility's capacity, use the External Referral tool to prepare the necessary documentation.
1. Creating an External Referral
From the Consultation room (/consultations/[id]):
- Navigate to the Outcome tab.
- Select Referred Out from the outcome options.
- Choose the Target Facility (e.g., Korle-Bu Teaching Hospital, Komfo Anokye).
- Provide the:
- Reason for referral.
- Clinical summary and findings.
- Provisional diagnosis.
- Any interim treatment provided.
- Click Process Referral.
2. Generating the Referral Form
Once processed:
- The referral is formally documented in the patient's record.
- You can print the official referral form from the Reports or Visit History section.
- The printed form contains all clinical notes to help the receiving facility.
[!NOTE] External referrals will close the current consultation as the patient is being transferred to another care provider.
[!IMPORTANT] The printed sheet MUST be signed by the Medical Officer in charge of the patient's care.
