External Referrals

When a patient needs care beyond your facility's capacity, use the External Referral tool to prepare the necessary documentation.

1. Creating an External Referral

From the Consultation room (/consultations/[id]):

  1. Navigate to the Outcome tab.
  2. Select Referred Out from the outcome options.
  3. Choose the Target Facility (e.g., Korle-Bu Teaching Hospital, Komfo Anokye).
  4. Provide the:
    • Reason for referral.
    • Clinical summary and findings.
    • Provisional diagnosis.
    • Any interim treatment provided.
  5. Click Process Referral.

2. Generating the Referral Form

Once processed:

  1. The referral is formally documented in the patient's record.
  2. You can print the official referral form from the Reports or Visit History section.
  3. The printed form contains all clinical notes to help the receiving facility.

[!NOTE] External referrals will close the current consultation as the patient is being transferred to another care provider.

[!IMPORTANT] The printed sheet MUST be signed by the Medical Officer in charge of the patient's care.
