Recording Vital Signs

Accurate vital signs are the foundation of a quality clinical assessment. This guide covers the triage workflow in EMR Ghana.

1. Accessing the Triage Queue

  1. Navigate to the Triage page (/triage) from the main menu.
  2. The queue displays all patients waiting for their vitals to be taken.
  3. Search for a patient by Name, Hospital Number, or Phone.
  4. Click on the patient's name to open the vitals capture form.

2. Capturing Vital Signs

Enter the measurements into the respective fields:

  • Blood Pressure (BP): Systolic and Diastolic values.
  • Pulse Rate: Beats per minute.
  • Respiratory Rate: Breaths per minute.
  • Temperature: Measured in Celsius.
  • SpO2: Oxygen saturation percentage.
  • Weight and Height: Used to auto-calculate the BMI.

3. Triage Categorization

Select the appropriate Triage Category to determine the urgency of care:

  • Emergency (Red): Immediate life-threatening condition.
  • Urgent (Yellow): Serious but not immediately life-threatening.
  • Standard (Green): Stable, routine clinical case.
  • Non-Urgent (White): Minor complaints.

4. Finalizing

  1. Add the patient's Chief Complaint (reason for visit).
  2. Click Save Vitals.
  3. The patient's status will automatically transition to With Doctor, and they will appear in the consultation queue.

[!TIP] Always verify the patient's identity using their Hospital Number before saving the vitals to the record.
